Pressing alt-shift-r will turn on the announcement of capital letters, format and attribute changes, all punctuation and blank lines. Pressing said hotkey a second time will revert to previously saved settings. In addition, these settings can be toggled individually with their own hotkeys, and the settings can be saved by pressing alt-shift-s. Press alt-shift-f1 to hear a complete list of the hotkeys used in this script and their descriptions. Press alt-shift-o to open a dialog which lets you change the settings without having to remember any hotkeys. If you press tab or shift-tab, you will go between the items. Press the spacebar or arrow keys to choose the settings.
